Abstract:

BACKGROUND: Pain after arthroscopic treatment can not only affect the patient’s life quality, and is not conducive to the early reasonable exercise and functional recovery of the patients after treatment. Up to 2012, there are 18 randomized placebo-controlled trials on intra-articular injection of bupivacaine for analgesia after arthroscopic surgery, but the results are different.
OBJECTIVE: To examine the efficacy and safety of intra-articular injection of bupivacaine in the management of pain after arthroscopic surgery through randomized placebo-controlled trials.
METHODS: The MEDLINE database, Cochrane Central Register and Google Scholar database were retrieved for the randomized controlled trials on intra-articular injection of bupivacaine in the management of pain after arthroscopic surgery up to April 2012. The key words were “bupivacaine, intra-articular, arthroscopic, postoperative pain, placebo”.
RESULTS AND CONCLUSION: Eighteen studies (n=934) were included (461 cases in bupivacaine group and 473 cases in the placebo control group). The Meta-analysis results showed the visual analog scale score of the bupivacaine group was lower than that of the placebo control group (weighted mean difference: -1.39, 95% confidence interval: -2.17 to -0.61, P < 0.000 01), and the number of patients required supplementary analgesia was less than the placebo control group (relative risk: 0.84, 95% confidence interval: 0.62 to 1.66, P=0.010). The time from first supplementary analgesia to postoperative intra-articular injection in the bupivacaine group was longer that in the placebo control group (weighted mean difference: 157.72, 95% confidence interval: 16.43 to 299.01, P < 0.000 01). There was no significant difference in the incidence of side effect between two groups (relative risk: 0.64, 95% confidence interval: 0.29 to 1.44, P=0.48). On the basis of the currently available literature, the intra-articular of bupivacaine after arthroscopic surgery can significantly relieve pain without increasing the adverse reactions when compared with the placebo control group.

2.1 基本情况及文献质量评价 根据检索策略，共检索出132篇文章，其中109篇与课题相关。经过初步筛选及随后的仔细检阅，筛选出38篇随机对照试验。再进一步阅读全文后，排除了其中的20篇，见图1。剩下18篇文献符合纳入标准^[4^，⁸^，¹¹^，^16-30]，最终纳入分析。

最终纳入18篇文献的基本信息见表1。

2.2 目测类比疼痛评分 10篇文献共报告了328例患者在治疗后不同时间点的疼痛情况(目测类比评分值)^[11^，¹⁷^，¹⁸^，^21-22^，^24-29]。总的来说，布比卡因组与对照组相比目测类比分值显著降低(WMD：-1.39，95%CI：-2.17至-0.61，P=0.000 05)。各研究间存在明显异质性(P < 0.000 01，I²=83%)，结果见图2。

2.3 需要补充镇痛药的患者数量 9个研究(10个试验)报告了需要额外镇痛药的患者数^[16^，^18-21^，²³^，^28-30]。Meta分析的结果显示布比卡因组和安慰剂组需要额外镇痛药的患者数差异无显著性意义(RR：0.84，95% CI：0.62 至 1.16，P=0.30)。研究间存在明显的异质性 (P = 0.01，I² = 59%)，结果见图4。

2.4 首次需要镇痛药距离关节腔内注射的时间 5个研究(7个试验)报告了首次需要镇痛药距离关节腔内注射的时间^[4^，¹¹^，¹⁶^，²¹^，²⁵^，^30]。合并后的数据显示布比卡因组的患者首次需要镇痛药距离关节腔内注射的时间明显大于对照组 (WMD：157.72，95%CI：16.43-299.01，P=0.03)。研究间存在明显的异质性(P < 0.000 01，I² = 100%)，结果见图6。

2.5 不良反应 4个研究提供了不良反应发生率的数据^[4^，¹¹^，¹⁶^，^30]。

数据合并分析显示，在关节腔内注射布比卡因组与安慰剂组相比较，不良反应的发生率差异无显著性意义(RR：0.64，95%CI：0.29-1.44，P > 0.05)。研究间无明显异质性(P > 0.05，I² = 0%)，结果见图8。

Begg漏斗图基本对称，无明显异常点，初步判断不存在发表偏倚。Begg秩相关检验也显示不存在明显的发表偏倚(P > 0.1)，见图9。

引言

材料方法

1.1 检索策略 Meta分析以PRISMA指南(Preferred Reporting Items for Systematic Reviews and Meta- analyses)为指导^[14]，通过检索Medline/PubMed、Cochrane library以及谷歌学术数据库，从最早至2012年4月有关关节镜治疗后关节腔内注射布比卡因与安慰剂对比的相关随机对照试验。

检索关键词包括“布比卡因，关节腔内，关节镜，术后疼痛，安慰剂”。

1.2 纳入标准与排除标准 第一作者与通讯作者2位研究者阅读了所有检索出的引文和摘要，每一位研究者单独评估所有的文献是否符合纳入标准。如果存在分歧，则通过讨论解决。

纳入标准：①所有的试验是随机的且以安慰剂为对照组的随机对照试验。②所有患者经历了关节镜手术。③所有患者在关节镜治疗后接受了布比卡因或安慰剂的关节腔内注射。④试验组的患者除了布比卡因没有接受其他镇痛药。⑤试验能提供可用的数据以计算合适的指标。

排除标准：①回顾性的或其他非随机对照试验研究。②试验组混合了除布比卡因以外的其他药物。③无法提取数据。

1.3 数据提取 2位研究者独立地提取纳入文献中的信息和数据。提取的纳入文献的信息包括：第一作者、发表的日期、地区、年龄、性别比、布比卡因的剂量、随访时间点、麻醉的类型、镇痛结果的测量以及不良反应。作者使用RevMan5统计软件来记录和处理提取的数据。

1.4 方法学质量评价 利用改良的JADAD量表来评价纳入研究的方法学质量。该量表总分为7分，主要从4个方面来评价随机对照试验：随机化方法、分配隐藏、盲法以及失访的情况。为了防止偏倚的发生，研究员在评价方法学质量时，不被告知杂志的名字、作者的名字以及他们的单位。

1.5 统计学分析 主要的分析结果是疼痛强度、需要额外镇痛药的患者的数量、首次需要镇痛药距离术后关节腔内注射的时间和不良反应发生率。

定量指标(目测类比评分，首次需要镇痛药离术后关节腔内注射的时间)计算合并加权均值差(weighted mean difference，WMD)及其95%的可信区间。

定性指标(需要额外镇痛药的患者的数量，不良反应发生率)计算合并相对危险度(relative risk，RR)及其95%的可信区间。

如果布比卡因使用不同的剂量，在分析中不同剂量的布比卡因所产生的结果当作一个独立研究。目测类比评分的测定在不同的研究中采用了不同的时间点，为了便于数据的整理并使之标准化，采用术后最后随访时间点的结果来进行分析。

各研究的异质性利用Q检验和I²统计量进行衡量，检验水准为α=0.05，并且以I²< 50%认为各研究之间不存在明显异质性。当各研究间异质性明显存在时，采用随机效应模型进行统计分析，反之则采用固定效应模型。

敏感性分析旨在检验根据不同的排除原则，将具有某些特殊特征的研究排除后对总体效应值的影响。运用Begg秩相关检验和漏斗图检验发表偏倚^[15]，检验水准为0.1。

采用RevMan 5和STATA version 11.0进行数据的统计分析。除了特殊指定的检验水准，其他所有统计分析中，检验水准α=0.05。
